The Harbor Consent Form
Required for all THS events. Valid from August 2022 to July 2023. If you do not know if we have a current form on file, email Athena Jeter.

Camp Cho-Yeh's Assumption of Risk Form
Required by Camp Cho-Yeh in order to attend and participate

Medication Form
Fill out this form if your student will need to take medication during the weekend. Put this form and the medication in a ziploc baggie labeled with the student's LAST NAME, FIRST NAME to present at check-in.
